Riboflavin is vitamin B2 — a water-soluble vitamin the body converts into two coenzymes, FAD and FMN.
Those coenzymes run energy metabolism, cell growth, and the handling of fats, drugs and steroids [1].
More than 90% of the riboflavin in food arrives already bound as FAD or FMN. The remaining tenth is free riboflavin plus glycosides and esters [1].
It is a genuine nutrient with an RDA and a real deficiency state, ariboflavinosis [1]. It is also sold at doses hundreds of times the RDA for migraine prophylaxis — a different use at a different scale, kept separate on this page [1][2].
Deficiency is extremely rare in the US. Fewer than 6% of the population falls below the Estimated Average Requirement [1].
What the Evidence Actually Supports

A 1998 randomized trial gave 55 adults either 400 mg/day or placebo for 3 months. 59% of the riboflavin group responded, versus 15% on placebo — an NNT of 2.3, or roughly two patients dosed per extra responder [2].
The American Academy of Neurology and American Headache Society graded riboflavin 'probably effective' and recommended offering it [1]. The Canadian Headache Society recommends 400 mg/day while rating the supporting evidence as low quality [1].

A 2021 review in Nutrients found plausible antioxidant, anti-inflammatory and mitochondrial pathways, but concluded there is 'no concrete evidence supporting its use' and called for larger trials [3].
Mayo Clinic's drug monograph is blunter still, saying claims of effectiveness for migraine are not established [9]. In ophthalmology, riboflavin drops applied to the cornea plus UV-A light is an established procedure to halt progression of keratoconus and other corneal ectasias [6][7].
A standard protocol runs about 30 minutes of UV-A at 3 mW/cm² [8]. The riboflavin acts as the photosensitizer that triggers new collagen cross-links, and as a UV shield for deeper eye structures [7].

Severe, prolonged riboflavin shortfall can produce anemia and cataracts, per NIH ODS [1].

Riboflavin is a coenzyme for several cytochrome P450 enzymes that process carcinogens, which is the theoretical hook for interest here [1].

Side Effects
- Bright yellow urine
High doses turn urine a vivid yellow-orange, and it startles people who were not warned.
It is simply the unabsorbed vitamin leaving in urine, where it fluoresces [1][11]. Every source here describes it as harmless and expected rather than a warning sign [1][11]. [1] - What the 400 mg/day trial reported
In the pivotal migraine trial, adverse events in the riboflavin arm were minor: diarrhea and increased urination [2].
Plus the yellow discoloration above. Nothing more serious surfaced at that dose over 3 months [2]. [2]
Precautions

Inadequate intake is not the only route into deficiency.
NIH ODS lists endocrine abnormalities such as thyroid hormone insufficiency, and certain diseases, as causes in their own right [1]. [1]
Interactions
- Whether interactions exist at all — the sources split
NIH ODS states plainly that riboflavin has no known clinically relevant medication interactions [1]. Mayo Clinic and WebMD likewise list no food or drug interaction requiring a dose change [9][16].
The Merck Manual Professional Edition disagrees. Its vitamin-drug table lists alcohol, barbiturates, phenothiazines, thiazide diuretics and tricyclic antidepressants as drugs affecting riboflavin status [12].
Some of the gap is definitional — 'needs a dose adjustment' versus 'measurably shifts nutrient status'. The disagreement about how much weight to give it is real either way. [1] - Oral contraceptives
Decades of academic work link oral contraceptive use to lower riboflavin status.
One study of 156 women found biochemical deficiency in 24 of 56 users, against 11 of 100 non-users, with rates climbing the longer use continued [13].
A 2024 review in npj Biofilms and Microbiomes offers estrogen or progesterone scavenging riboflavin as a possible mechanism [14]. The same review flags an unsubstantiated hypothesis tying pill-associated migraine to reduced plasma riboflavin [14]. [13]
Food Sources
Organ meat and fortified foods lead by a wide margin. Pan-fried beef liver delivers 2.9 mg per 3 oz, or 223% of the Daily Value [1].
Fortified breakfast cereal gives 1.3 mg a serving (100% DV), fortified instant oats 1.1 mg per cooked cup [1]. Plain fat-free yogurt has 0.6 mg a cup, 2% milk 0.5 mg, lean beef and clams 0.4 mg per 3 oz, almonds and Swiss cheese 0.3 mg a serving [1].
Mushrooms, chicken, eggs, quinoa, salmon, spinach and whole grains add smaller but real amounts [1].
Riboflavin survives heat but not water or light. Boiling leaches roughly twice as much into the cooking water as steaming or microwaving does, and the same light-sensitivity is why milk ships in opaque containers [1][3].
US adults average 4.5 mg/day (men) and 4.7 mg/day (women) from food plus supplements, well above the RDA [1]. NHANES found lacto-ovo vegetarians slightly ahead of non-vegetarians on dietary intake — 2.3 against 2.1 mg/day [1].
Harvard's Nutrition Source and NIH ODS still flag vegans and others eating little dairy or meat as at risk, since animal foods remain the most concentrated natural sources [1][15].
Best Time to Take
No government or academic source establishes an optimal time of day, and NIH ODS does not address timing at all [1].
Cleveland Clinic's tablet monograph says to take it by mouth with a glass of water, with meals, at regular daily intervals [11]. Healio's clinical reference similarly advises consistent daily timing, without requiring food [17].
There is no known food-drug interaction, so with or without food both work [1][16].
One open question sits underneath all this. Absorption saturates near 27 mg per dose, so a single 400 mg capsule mathematically leaves most of itself unabsorbed [1].
No published migraine trial compared split dosing against one daily dose. So nothing actually confirms that dividing it helps [1][2].
Common Questions
Who is actually at risk of running low on riboflavin?
Deficiency is extremely rare in the US, but a few groups sit closer to the line [1].
Vegetarian athletes, because exercise raises riboflavin turnover. Vegans and others who eat little dairy or meat [1].
Pregnant and lactating women with low meat and dairy intake, along with their infants [1]. Maternal intake below 1.2 mg/day is linked to higher infant deficiency and to certain birth defects, including cardiac outflow-tract defects [1].
Low riboflavin status in pregnancy has also been linked to increased preeclampsia risk [1].
Why do the US and Canadian recommended intakes differ?
The US RDA is 1.3 mg/day for men and 1.1 mg/day for women [1]. Canada's Recommended Nutrient Intakes run roughly 1.0–1.6 mg/day for males and 1.0–1.1 mg/day for females, per Mayo Clinic's cross-reference table [9].
That is two national bodies setting standards slightly differently. It is not a scientific dispute about the vitamin itself [1][9].
How is vitamin B2 different from vitamin B12?
Both are water-soluble B vitamins involved in energy metabolism and red blood cell production, but they are chemically distinct nutrients [1][18].
B2 deficiency, ariboflavinosis, shows up as mouth and lip sores and skin complaints. B12 — found almost exclusively in animal products — has its own well-known picture of anemia and neurological problems [1][18].
Is riboflavin safe during pregnancy and breastfeeding?
At the recommended intakes — 1.4 mg/day in pregnancy, 1.6 mg/day while breastfeeding — riboflavin is both safe and important [1].
The high-dose question is separate. A 2019 review of high-dose vitamins concluded riboflavin's wide safety margin is not itself a reason to discontinue breastfeeding, even at the 400 mg/day migraine dose [19].
That dose remains a clinical decision. It belongs with whoever manages your care, not with a bottle label [19].
How should riboflavin supplements be stored?
Closed container, room temperature, away from heat and moisture and direct light, and don't let it freeze [9].
The direct-light part is not boilerplate. Riboflavin is genuinely light-sensitive, more so than most vitamins on the shelf [1][9].
