The real ceiling is your gut. Shah and Limketkai's clinical review puts it at 50–100 g/day — 4–7 tablespoons, or 60–100 mL — divided across meals [1].

A second limit is nutritional rather than toxic. MCTs carry no essential fatty acids, so using MCT as your only fat for more than about 3 weeks calls for adding an oil that supplies linoleic and linolenic acid [1].

MCT stands for medium-chain triglyceride.

In practice, an MCT oil bottle means caprylic acid (C8) and capric acid (C10) — the two chain lengths that absorb fastest and raise ketones best [2].

Caproic acid (C6) is left out for its taste and smell. Lauric acid (C12), the fatty acid that dominates coconut oil, is metabolized more like a long-chain fat and is often excluded or present only in traces [2].

So coconut oil is not MCT oil, and cooking with one is not the same as dosing the other [2].

The product is manufactured, not harvested: medium-chain fatty acids are hydrolyzed off coconut or palm kernel oil, purified, then re-esterified onto glycerol [1].

  • Digestive upset — the common one, and it is dose-dependent

    Nausea, vomiting, stomach pain, gas, cramping, bloating and diarrhoea are the documented effects [7].

    They show up mainly at higher doses, when the dose ramps too fast, or when it lands on an empty stomach [1].

    They usually settle as the amount is raised gradually, which is why the starting dose is a teaspoon rather than a tablespoon [1]. [7]

Precautions

  • Liver disease is the significant contraindication

    US drug-reference labeling lists cirrhosis, other liver disease, portal hypertension, hepatic encephalopathy and a previous portacaval shunt as reasons to check with a doctor or pharmacist first [10].

    A Cleveland Clinic gastroenterologist advises against MCT oil outright for people with fatty liver disease or heart disease [9].

    Sustained intake well above normal supplemental amounts has been linked to fat accumulating in the liver in some research [2][9]. [10]

    WebMD's medication guidance flags valproate-family seizure drugs — divalproex sodium, valproate sodium, valproic acid — plus triheptanoin (Dojolvi, itself a prescription MCT-based drug) and the weight-loss drug orlistat [16].

    The sources disagree on whether interactions exist at all. Healthline states there are no reported adverse medication interactions [2].

MCT oil is a manufactured concentrate rather than a food — medium-chain fatty acids are extracted and isolated from coconut or palm kernel oil, then re-esterified [1].

As whole foods, the richest natural sources are coconut oil at about 55% medium-chain fat and palm kernel oil at about 54%, on USDA FoodData Central figures [2].

Dairy fat is far behind: whole milk around 9%, butter around 8% [2].

Smaller amounts turn up in cheese, yogurt, goat's milk and dark chocolate. Human breast milk contains them naturally too [7][9].

The caveat that matters: coconut oil's medium-chain share is mostly lauric acid (C12), which behaves metabolically like a long-chain fat, so a spoon of it is not a spoon of C8/C10 oil [2].

No clinical trial has established a best time of day — the sources describe habits, not tested protocols [1].

Shah and Limketkai's guidance is about spreading rather than timing: divide the daily amount evenly across meals, roughly 1–2 tablespoons per meal on a three-meal day, for gut tolerance [1].

On fasted versus fed, there is a genuine trade-off. A small 2023 study found MCT oil moved insulin sensitivity and ketone markers more when taken without food [2].

Can I cook or fry with MCT oil?

Not at high heat. MCT oil has a relatively low smoke point, and clinical dosing guidance says specifically not to heat it above 150°C (302°F) [1].

Past that, flavour suffers and oxidation risk climbs [1].

It suits mixing into food and drink, low-heat cooking, or drizzling on afterwards. It is liquid at room temperature and largely colourless and tasteless, so smoothies, juices, soups, cereal, coffee and tea all work [1].

Does MCT oil go off, and how should I store it?

It can degrade, though the reviewed sources give no single authoritative shelf life for consumer bottles — the product's own best-by date is the figure to use [10].

Drug-reference guidance is room temperature, away from moisture and heat [10].

Chemically it holds up better than most cooking oils, because fully saturated fatty acids resist rancidity during storage, per food-science research [17]. It can still oxidize with long exposure to heat, light and air once opened [17].

Does MCT oil break a fast?

That is more a definitional question than a clinical one, and no trial in these sources tests it directly.

What is established: MCT oil is calorie-dense at about 115 kcal per tablespoon, so it breaks a strict zero-calorie fast [1].
