HMB is short for beta-hydroxy-beta-methylbutyrate, and your body already makes it.

It is a downstream metabolite of leucine, the branched-chain amino acid [2][3].

The route runs leucine → alpha-ketoisocaproate (KIC), mostly via BCAA aminotransferase in skeletal muscle, then KIC → HMB via KIC dioxygenase, mostly in the liver [2][3].

Only about 2–10% of leucine metabolism ends up as HMB, so what the body makes on its own is limited [2][3]. Fasting plasma levels sit around 1–4 µM, rising roughly 5- to 10-fold after a leucine-rich meal [2].

It is sold to reduce exercise-induced muscle breakdown. It is not an essential nutrient, and no deficiency state for it is described in these sources [2][3].

    The ISSN position stand concludes that 38 mg/kg body mass daily, paired with appropriate training, enhances muscle hypertrophy, strength and power [1].

    It also credits HMB with increased lean body mass and functionality in elderly, sedentary populations, and possible fat-mass reduction when combined with structured exercise [1]. [1]

  • Trained and competitive athletesNot shown to help

    The picture inverts once training status rises.

    An analysis of 6 studies (193 participants) in trained athletes, at 3–6 g/day for anywhere from 3 days to 12 weeks, found no improvement in strength or body composition [5].

    The proposed explanation is mechanical, not dismissive: strength-trained muscle already has suppressed proteolysis from training adaptation, leaving HMB less to work on [3]. [5]

  • Older adults with sarcopeniaPossibly helps

    ⚠ Two credible bodies of evidence, pointing different ways.

    A 2023 randomized double-blind placebo-controlled trial gave HMB plus resistance training to older adults with sarcopenia for 12 weeks [7]. Handgrip strength rose 4.61 kg, gait speed 0.11 m/s, and five-time chair-stand time fell 3.65 seconds against placebo [7].

    The inflammatory marker TWEAK also fell. Total skeletal muscle mass did not significantly differ — the gains were functional, not bulk [7].

    Against that sits a 2019 review of 10 studies in 384 adults aged 50+ [3]. It found little to no effect on body composition, strength or performance when HMB was added to an exercise program rather than exercise alone [3]. [7]

    Worth stating plainly, since it explains most of the disagreement above.

    Benefit tracks training status. Untrained, elderly and clinical populations are where positive results cluster; already-trained athletes are where they vanish [1][3][5].

    A marketing page written for athletes is therefore quoting evidence collected largely in people who are not athletes. [1]

  • Muscle wasting in clinical conditionsPossibly helps

    A review describes compelling evidence that HMB may be useful in cancer, AIDS, bed rest and caloric deficit [1].

    The catch is study design. Few controlled trials isolate HMB on its own rather than as part of a combination therapy, so the individual contribution is hard to read [1]. [1]

    A GRADE-assessed 2025 meta-analysis of controlled trials found HMB significantly raised testosterone in adults, at a standardized mean difference of 0.82 (95% CI 0.35–1.29) [12].

  • Allergic reaction — rare, but a stop signal

    WebMD's monograph lists rash, swelling, difficulty breathing and hives as reasons to stop HMB and seek care [6].

  • Digestive complaints

    Stomach pain, constipation and itching have been reported in trials, described as rare [6].

    Nine pooled human studies at 3 g/day found no adverse effects on blood chemistry, hematology, or organ and tissue function markers [14]. [6]

Precautions

  • Insulin sensitivity — a rodent signal, unchecked in people

    ⚠ Flagged as an open question, not an established human risk.

    One rodent study at 320 mg/kg body mass daily for a month found raised fasting plasma insulin, which points toward reduced insulin sensitivity [1].

    The ISSN position stand notes this has not been reported in any human study to date, and calls for human research on exactly this [1]. [1]

  • Cholesterol and blood pressure — the sources split

    ⚠ Both readings stay visible, because one is old and pooled and the other is recent and mixed.

    A pooled analysis of 9 human studies at 3 g/day for 3–8 weeks found total cholesterol down 5.8%, LDL down 7.3%, and systolic blood pressure down 4.4 mmHg against placebo [14].

    A 2025 systematic review of lipid effects found the trial-level picture more mixed, with several trials showing no significant change in LDL, HDL or total cholesterol [15].

    That review's suggestion is that any LDL reduction may appear only in people who start with elevated cholesterol, not in those with normal levels [15]. [15]

  • Pregnancy and breastfeeding

    Not established, in either direction. WebMD states it is not known whether HMB affects pregnancy or a fetus, or whether it passes into breast milk [6].

    A peer-reviewed spiny-mouse study concluded that using HMB in human pregnancy needs further experimentation for fetal safety [13].

    It cites other animal work where prenatal HMB exposure altered offspring hormone levels and bone development [13]. [13]

Interactions

  • mTOR-inhibitor drugs such as rapamycin

    Memorial Sloan Kettering's integrative medicine reference flags a theoretical, laboratory-level interaction [16].

    HMB stimulates the mTOR pathway, and that stimulation was reduced by mTOR inhibitors in lab studies [16]. Whether HMB meaningfully changes how those drugs work in people is not known [16]. [16]

Food Sources

HMB occurs naturally in a short and slightly odd list of foods: alfalfa, corn including corn silage, milk, cheese, grapefruit, catfish, red wine and red meat [10].

The amounts are trace. HMB is chiefly a metabolite of leucine, which comes from high-protein foods such as meat, dairy, soy and legumes [2][3].

And the conversion is inefficient — only about 2–10% of leucine metabolism produces HMB [2][3].

Diet plus the body's own production therefore falls well short of the roughly 3 g/day used in trials. That gap is the entire reason HMB is sold as a concentrate [2].

Best Time to Take

There is no single agreed protocol, but the trial record splits into two patterns.

For chronic benefit, most trials simply split HMB-Ca across three daily doses at breakfast, lunch and dinner, with no timing relative to exercise [1]. That produced measurable strength and body-composition changes over several weeks [1].

For an acute pre-workout effect, the ISSN recommends 3 g of HMB-Ca at least 60 minutes before intense exercise [1]. Stretch that to 120 minutes if it is taken with glucose, which slows absorption [1].

The faster-absorbing free-acid form is recommended instead at 1–2 g, 30–60 minutes beforehand [1].

One controlled study tested timing directly. A 3 g HMB-Ca bolus 60 minutes before eccentric leg extensions blunted the rise in LDH, a muscle-damage marker, and tended to reduce quadriceps soreness [1].

The same dose taken immediately after exercise did nothing measurable, nor did pre-exercise placebo [1]. For a demanding training block, ISSN's advice is to start the daily 3 g about two weeks ahead [1].

Common Questions

How long before HMB does anything measurable?

Weeks, not days, and the markers move in sequence.

The original dose-response work in previously untrained people found HMB-Ca reduced markers of muscle protein breakdown more at two weeks than at one [1].

Creatine kinase, a muscle-damage marker, did not fall significantly until the third week of training [1].

Measurable fat-free-mass and strength gains have been reported within three weeks of 3 g/day plus resistance training, again in untrained people [1].

Do you need to cycle HMB, or load it like creatine?

Neither, on the evidence here.

No loading protocol for HMB appears in these sources, unlike creatine's familiar front-loaded phase [1]. Most trials simply ran continuous daily dosing for 3 to 12 weeks [1].

ISSN's practical advice is timing rather than cycling: begin the regular dose about two weeks before a demanding block [1].

Safety data reach about one year of continuous daily use in elderly subjects, with no adverse changes in blood or urine liver and kidney markers [1]. Whether it keeps working over that span is a separate, unstudied question.

Does HMB break a fast?

Not in the sense the fasting research was set up to measure, though the strict autophagy question was never tested here.

A controlled study used 3 g/day of free-acid HMB across a 3-day meat-free diet and a following 24-hour fast [11].

It found no effect on the urinary marker of muscle protein breakdown — the 3-methylhistidine to creatinine ratio — versus placebo [11].

The researchers were comfortable running HMB inside a fasting protocol on that basis [11].

Does HMB cause hair loss or change your hormones?

On hair loss: nothing. None of the sources reviewed measured or reported hair loss as an outcome, so there is no answer here either way.

On hormones, there is real data. A 24-hour fasting study found free-acid HMB blunted the normal morning cortisol rise in men [11].

It raised the testosterone-to-cortisol ratio by 162% during that fast [11]. That is a ratio shift under fasting stress, not a general statement about resting hormone levels.

HMB or creatine — what is the actual difference?

Different compounds, different jobs.

Creatine is built from glycine, arginine and methionine, and works mainly by resupplying the phosphocreatine energy system for short, high-intensity efforts [5].

HMB is a leucine metabolite that works primarily by reducing muscle protein breakdown, which is an anti-catabolic role rather than an energy one [5].

Some studies combining 3 g of HMB with creatine in already-training athletes reported added gains in strength, high-intensity performance and body fat over either alone [5]. At least one, in rugby players, found no advantage to combining them [5].

Why take HMB rather than just leucine?

Because HMB is what a small slice of your leucine turns into anyway — roughly 2–10% of it [3].

A review of HMB and skeletal muscle proposes that leucine's own effects on muscle protein balance may work partly through that conversion [3].

It suggests HMB supplementation may reduce how much leucine is needed for a similar effect [3].

⚠ No head-to-head comparison of equivalent HMB and leucine doses on muscle protein synthesis appears in these sources. The case is mechanistic, not measured [3].
