Adverse events were far less likely when peak plasma carnosine stayed below about 150 μM, roughly a dose under 100 mg/kg body weight [2].

⚠ A narrative review states there is no risk of overdose, on the grounds that carnosinase cleaves carnosine into harmless amino acids [1]. The dose-escalation data contradicts that in practice [2].

Carnosine is a dipeptide — two amino acids, beta-alanine and histidine, joined together [1].

The body makes its own from those two building blocks, estimated at roughly 427 mg/day for a 60 kg woman and 606 mg/day for a 70 kg man [1].

There is no dietary requirement for it, and no recognized carnosine deficiency syndrome [1][5].

In the US it is sold as a dietary supplement, so the FDA has not reviewed it for safety or effectiveness before sale [7].

    This is carnosine's best-supported use. Multiple RCTs at 500 mg–2 g/day for 8–12 weeks in prediabetes and type 2 diabetes report better insulin sensitivity and secretion [1][3].

    Fasting glucose, HbA1c, triglycerides and TNF-alpha all came down across those trials [1][3].

    In diabetic nephropathy, a nephroprotective effect showed up as reduced urinary TGF-beta [1]. [3]

  • Cognitive declinePossibly helps

    A 2026 systematic review and meta-analysis rated the evidence only low strength [4].

    It found carnosine — often combined with anserine or other antioxidants — may slow cognitive decline in healthy elderly people, or in those with probable Alzheimer's or mild cognitive impairment [4].

    The reviewers explicitly called for more high-quality RCTs [4].

  • Athletic performance, from oral carnosine itselfPossibly helps

    Oral carnosine is not what athletes reach for, and the reason is pharmacological. Circulating serum carnosinase breaks much of it down before it reaches muscle [1][7].

    Beta-alanine, carnosine's rate-limiting precursor, is generally considered the more effective route to raising muscle carnosine stores [1].

    One small trial of oral carnosine itself — 6,400 mg/day for 10 weeks, n=26 — found no significant fitness difference versus placebo [1]. [1]

  • Zinc-L-carnosine (polaprezinc)Likely helps

    The most clinically established application belongs to a different compound: zinc-L-carnosine, a synthetic zinc and carnosine chelate [1].

    It is officially approved in Japan and South Korea for gastric ulcer therapy and as support for H. pylori eradication [1].

    In the US, Canada and Australia it is approved for chemotherapy- and radiation-induced oral mucositis and taste disorders [1]. None of that transfers to plain oral carnosine. [1]

  • Cataract eye drops (N-acetylcarnosine)Possibly helps

    ⚠ Sharply split evidence, with a conflict of interest sitting in the middle of it.

    The human-trial results come almost entirely from a single research group that also holds the patent on the treatment [13].

    An independent Cochrane review could not include those trials, because they did not report enough detail [13].

    Cochrane's conclusion: there is no convincing evidence that N-acetylcarnosine drops slow or undo cataract progression [13]. [13]

  • Acute adverse events

    Headache, nausea and vomiting, paraesthesia, hypotension and flushing were the reported effects, all mild to moderate [2].

    They mostly emerged 1 to 4 hours after dosing [2]. [2]

Precautions

  • Severe liver or kidney disease

    The dose-escalation study found no abnormal liver or kidney blood results at a single 10 g dose [2].

    But maximum safe doses specifically in people with severe liver or kidney disease have never been formally established by any dedicated study [5]. Caution is the standing advice. [5]

  • Pregnancy and breastfeeding

    No source reviewed here demonstrated actual harm [6][7].

    Multiple independent consumer-medical references still advise avoiding carnosine while pregnant or breastfeeding, purely because dedicated safety data does not exist [6][7]. [6]

  • Carnosinemia

    Carnosinemia, or serum carnosinase deficiency, is a rare autosomal recessive disorder where the CNDP1-encoded enzyme fails to break carnosine down [9].

    It raises carnosine in blood, urine and cerebrospinal fluid — the opposite of a deficiency [8][9].

    Reported symptoms range from none at all to developmental delay, intellectual disability, hypotonia, tremors, seizures and sensory or axonal neuropathy [9].

    ⚠ Severity does not track residual enzyme activity, and one original case series questioned whether the neurological symptoms come from the enzyme defect at all [10]. [9]

  • Everything else is unmapped

    Dedicated interaction research on carnosine is sparse [7].

    One consumer medical reference states plainly that interactions between carnosine and other medicines are not fully understood [7]. [7]

Interactions

  • Blood pressure medication

    Oral carnosine lowered blood pressure at every dose tested in the human dose-escalation study — 8.7 mmHg systolic and 10.7 mmHg diastolic at the 4 g dose [2].

    The largest drops landed 5 to 6 hours after dosing [2].

    A pharmacy reference rates carnosine with antihypertensives — amlodipine, nifedipine, verapamil, diltiazem — as a moderate interaction, since the combination could push blood pressure too low [6]. [2]

Food Sources

Chicken, fish and shrimp are the main quantifiable dietary sources, and beta-alanine comes mostly from poultry, beef and fish [1].

Measured values swing hard by species, breed, muscle type and age.

An HPLC analysis of four Thai native chicken breeds put breast meat at 423–578 mg/100g, against a broader literature range of 130–798 mg/100g [14].

A Korean native chicken study found raw breast at 160–201 mg/100g and leg or dark meat at only 55–88 mg/100g [15]. White, fast-twitch fibers carry more carnosine than red, dark ones [15].

Cooking costs you some, and the method decides how much: boiling beef loses roughly 50%, microwaving about 20%, microwave-then-grill only about 10% [12].

Carnosine is highly water-soluble, which is why boiling is the worst of those [12].

In the Korean chicken study, boiling took breast meat from about 127 mg/100g raw to about 99 mg/100g — a 22% reduction [15].

One review estimates about 30 g of dried beef would supply a 70 kg adult's whole daily self-synthesized equivalent, roughly 600 mg [1]. It is a single-source estimate, and the body does not require dietary carnosine anyway.

Vegetarians measure about 26% lower muscle carnosine than omnivores [11].

⚠ The same review lists asparagus, green peas and white mushrooms as sources [1]. That traces to one old conference abstract, sits against the vegetarian data, and plant tissue lacks the muscle biosynthetic pathway — so we are flagging it as unverified rather than listing it as fact.

Best Time to Take

No timing study on carnosine appears to exist, and nothing compares morning against evening dosing [2].

What is established is the pharmacokinetics: fasted, oral carnosine peaks in plasma within 15 minutes to 1 hour depending on dose, with little left circulating past about 4 hours [2].

The study authors flag that absorption could plausibly be slower in a non-fasted state, since carnosine would compete with other nutrients for intestinal transporters [2].

That was a caveat about their own fasted-state protocol, not something they tested [2].

Across the clinical trials, the practical pattern is simply splitting the daily dose in two [1][2].

Common Questions

Is it safe to take carnosine every day, long term?

The only long-term human data is a 4-week sub-study of 5 g taken twice daily, which produced no adverse events and no abnormal blood results [2].

That sub-study had 4 participants [2].

Nothing in the reviewed literature covers daily use beyond 4 weeks, so longer-term use is not backed by trial evidence either way [2].

Does carnosine do anything for aging skin and wrinkles?

Carnosine is marketed as an anti-aging nutrient on the strength of its antioxidant and anti-glycation chemistry [5].

An independent review found the quality of the existing human studies too low to give reliable evidence of a real benefit [5].

So: a plausible mechanism, without human results good enough to stand on.

Is carnosine an amino acid or a peptide?

A peptide — specifically a dipeptide, built from exactly two amino acids linked together [1].

Those two are beta-alanine and histidine [1]. It is not a single amino acid itself, and it is far shorter than the chains that make up typical proteins.

Carnosine vs carnitine — what is the difference?
