What is documented is a practical ceiling. At 3 g/day and above, supplemental carnitine commonly causes nausea, vomiting, abdominal cramps, diarrhea and a fishy body odor [1][2].
Higher medical doses of 20–200 mg/kg/day exist, but only for confirmed carnitine deficiency under clinical supervision [1].

Carnitine is built from the amino acids lysine and methionine, and the body makes its own supply in the liver, kidneys and brain [1].
Its main job is hauling long-chain fatty acids into the mitochondria to be burned for energy. It also clears certain toxic compounds out of mitochondria [1].
About 95% of the body's carnitine sits in heart and skeletal muscle [1].
It is not an essential nutrient. The Food and Nutrition Board reviewed this in 1989 and concluded healthy children and adults need no carnitine from food or supplements at all [1].
A prescription drug form also exists — levocarnitine, FDA-approved and dosed as medicine rather than as a supplement [10].
What the Evidence Actually Supports
- Alzheimer's disease and dementiaPossibly helps
A 2003 meta-analysis of 21 trials found benefits from 1.5–3.0 g/day of acetyl-L-carnitine [1].
A 2003 Cochrane Review covering overlapping trials found symptom improvement at 12 and 24 weeks but not at 52, and concluded routine clinical use was not justified [1].

A meta-analysis of 13 trials in acute myocardial infarction patients (2.7–6 g/day) found lower all-cause mortality, fewer ventricular arrhythmias and less new-onset angina [1].

A meta-analysis of 3 trials (201 infertile men) found sperm motility up 7.84% and morphology up 4.91%, with no change in sperm concentration [1].

Signs of Low Levels
- Primary carnitine deficiency — a rare inherited defect in the OCTN2 cellular carnitine transporter (the SLC22A5 gene), usually appearing in infancy or early childhood [1][7]
- In infants it can cause epilepsy and encephalopathy
- In adolescents and young adults: seizures, irregular heartbeat and breathing problems
- In older individuals: myopathy, rhabdomyolysis, cardiomyopathy or sudden death
- Even carriers without symptoms carry a raised lifelong risk of heart failure, liver disorders and coma
- Secondary carnitine deficiency — driven by something else, such as chronic renal failure, or long-term use of pivalate-containing medications
- Its signs: hyperammonemic encephalopathy (malaise, seizures, reduced consciousness from high blood ammonia), hypoglycemia, low blood ketones, dicarboxylic aciduria and raised uric acid
- Also muscle weakness, myoglobin in the urine, cardiomyopathy and sudden death
- Highest-risk groups: premature infants, plus people with end-stage renal disease or on hemodialysis, where low carnitine can contribute to anemia, muscle weakness, fatigue and altered blood lipids
- Both forms are corrected with high-dose supplemental carnitine, 20–200 mg/kg/day, under medical supervision [1]
- Neither is something a healthy adult develops from diet — the liver and kidneys make enough [1][1]

Side Effects
- Doses at 3 g/day and up
Nausea, vomiting, abdominal cramps, diarrhea and a fishy body odor are the documented effects at 3 g/day or higher [1][2].
Milder gastrointestinal upset gets reported at lower doses too [2]. [1]
Precautions
- Uremia and seizure disorders
Carnitine can trigger muscle weakness in people with uremia [1].
In people with seizure disorders it can trigger seizures [1]. Both are specific clinical populations, not general warnings. [1] - Thyroid
Carnitine acts as a peripheral antagonist of thyroid hormone, blocking T3 and T4 entry into cell nuclei [9].
A randomized double-blind placebo-controlled trial used 2 or 4 g/day against symptoms of iatrogenic hyperthyroidism in women on suppressive levothyroxine [9].
⚠ The widespread online warning to avoid carnitine with an underactive thyroid does not trace to any trial in hypothyroid patients [9].
It extrapolates from the antagonist mechanism. Worth raising with a prescriber, but it is not a documented harm. [9]
Interactions
- Anticonvulsants
Valproic acid, phenobarbital, phenytoin and carbamazepine all lower blood carnitine levels [1].
Valproic acid in particular can cause hepatotoxicity and raised ammonia, and intravenous L-carnitine is given for valproic acid toxicity [1]. [1] - Pivalate-conjugated antibiotics
Chronic use of pivalate-conjugated antibiotics such as pivampicillin depletes carnitine [1].
No case of resulting illness has been documented, so this is a measurable depletion without a known clinical consequence [1]. [1] - Warfarin and acenocoumarol
A major drug-interaction reference lists the prescription form, levocarnitine, with warfarin or acenocoumarol as usually not recommended [10].
It allows the combination with dose adjustment where necessary [10]. [10]
Food Sources
Carnitine concentrates in animal foods, and red meat leads by a distance. Plant foods carry traces [1].
Per 3-ounce cooked serving: beef steak 42–122 mg, ground beef 65–74 mg, codfish 3–5 mg, chicken breast 2–4 mg [1].
Elsewhere: 1 cup whole milk 8 mg, ½ cup ice cream 3 mg, 2 oz cheddar 2 mg, 2 slices whole-wheat bread 0.2 mg, ½ cup cooked asparagus 0.1 mg [1].
Food carnitine absorbs at 63–75%, far better than supplemental L-carnitine's 14–18% [1].
For a 165-lb person, an omnivorous diet supplies about 24–145 mg/day; a vegan diet supplies about 1.2 mg/day [1].
That gap matters less than it looks. The same person synthesizes roughly 14.4 mg/day unaided, and strict vegetarians draw 90% or more of their carnitine pool from synthesis rather than food [1][7].
Best Time to Take
There is no established time-of-day guidance for carnitine, which follows from it not being an essential nutrient with a set intake schedule [1].
The one relevant pharmacokinetic fact: plasma L-carnitine peaks about 3.4 hours after a single oral dose, later than its shorter-acting ALC and PLC derivatives [3].
The specific rules you find online — empty stomach before training, between meals with coffee — come from commercial and practitioner blogs, not from trials [3]. We are leaving them off.
Common Questions
Is carnitine safe while pregnant or breastfeeding?
Not established either way, which is not the same thing as safe.
For breastfeeding, NIH's LactMed found no published safety or efficacy data in nursing mothers, or in infants without a diagnosed deficiency [6].
It reasons that poor bioavailability limits what an infant would absorb from breastmilk, concluding maternal supplementation is unlikely to harm the infant but is best avoided unless a clinician prescribes it [6].
For pregnancy specifically, no government or academic source reviewed here established general safety data [6].
Do vegetarians and vegans need a carnitine supplement?
Vegans take in about 1.2 mg/day from food against 24–145 mg/day on an omnivorous diet, and still are not typically deficient [1].
Own-body synthesis covers it — strict vegetarians derive over 90% of their carnitine pool from their own production [7].
Studies do find modestly lower plasma carnitine in vegetarians, and supplementation measurably raises both plasma and muscle stores [8].
One 12-week trial raised vegetarians' muscle carnitine by about 13% [8]. That is a lower baseline moving up, not a deficiency being corrected.
How should I store L-carnitine, including the injectable form?
The FDA-approved oral drug label specifies room temperature (25°C/77°F), kept in its carton away from light, avoiding excessive heat and freezing [5].
Unmixed injectable levocarnitine vials are stored at room temperature and must not be frozen [14].
Once mixed and diluted, that solution can sit at room temperature for up to 24 hours [14].
Over-the-counter capsules, tablets and liquids follow the same basic principle, though this guidance comes from the prescription labelling rather than a supplement-specific source [5].
How is carnitine different from creatine?
They get confused because both are amino-acid-derived, sold as sports supplements, and found in red meat [13].
Creatine is stored in muscle mainly as phosphocreatine, a rapid-response buffer that regenerates ATP during short, high-intensity efforts [1].
Carnitine instead shuttles long-chain fatty acids into mitochondria for sustained energy production [1]. Different energy system, different job.
