Unlike strawberries and blackberries, the blueberry is a true botanical berry — one ovary, fleshy pericarp, many seeds [22][23].
It belongs to Vaccinium in the heather family, and the compounds under study are its anthocyanins, the pigments that make it blue [20].
As a supplement it appears in four quite different guises: freeze-dried whole-fruit powder, standardized anthocyanin extract, juice or concentrate, and capsules [9]. Those are not interchangeable, which turns out to matter for the results.
Blueberry supplements have not been reviewed by the FDA for safety or effectiveness, so label instructions are the manufacturer's own [10].
What the Evidence Actually Supports
- Memory and cognition in older adultsPossibly helps
A 2022 systematic review of 11 randomized controlled trials in older adults found berry supplementation associated with better resting brain perfusion, memory, executive function, processing speed and attention [1]. Effects varied by test and by study.
A much-cited trial of just 9 older adults with early memory decline used wild blueberry juice for 12 weeks, improving paired-associate learning (p=0.009) and word-list recall (p=0.04) [2].
A later crossover trial found 111–444 mg of wild blueberry extract improved episodic memory within 2–6 hours, notably during the post-lunch dip [3]. The highest dose tested, 888 mg, did no better than the lower ones. [1] - Blood pressure — sources directly conflictPossibly helps
This one is a live disagreement and we are not settling it.
A 2017 meta-analysis of 6 RCTs (204 participants) found no significant effect on systolic or diastolic pressure, netting -0.28 mmHg (95% CI -1.11 to 0.56) [4].
Yet an RCT in 40 healthy men found 200 g/day of whole wild blueberries for a month lowered systolic pressure by about 5 mmHg [5]. A metabolic-syndrome trial using 45 g/day of powder found no blood-pressure change at all [6].
Dose, form and starting population all differ across those studies, which is the most likely explanation [4][5][6]. [4] - Endothelial functionPossibly helps
The most consistent cardiovascular signal in these trials is vessel function rather than blood pressure.
In healthy men, 200 g/day of wild blueberries improved flow-mediated dilation by about 2%, seen within 2 hours and sustained after a month of daily intake [5].
In adults with metabolic syndrome, 45 g/day of freeze-dried powder for 6 weeks significantly improved the reactive hyperemia index against placebo [6]. Both trials were small. [5] - Insulin sensitivity and blood sugarPossibly helps
Findings here are inconsistent and mostly underpowered.
The 6-week metabolic-syndrome RCT found no change in insulin sensitivity from blueberry smoothies [6].
A 6-month pilot gave 15 elderly patients with type 2 diabetes 400 mg/day of anthocyanin extract [8]. It reported favourable but not statistically significant trends in fasting glucose and HbA1c, plus a measurable shift in gut microbiota.
Animal and cell studies describe anthocyanins raising insulin sensitivity and altering lipid-metabolism gene expression, but reviewers caution this has not carried over to human RCTs [9]. [6] - Blood lipids and cardiovascular riskPossibly helps
A 2021 meta-analysis pooling 44 RCTs and 15 prospective cohort studies on anthocyanin-rich berries found lipid-lowering and anti-inflammatory associations, alongside lower cardiovascular disease risk [7].

In the Women's Health Study, a prospective cohort of over 36,000 women, higher anthocyanin intake was modestly associated with lower cataract risk [18].
In the same analysis, neither blueberry nor anthocyanin intake was independently associated with age-related macular degeneration risk — while blueberry intake itself showed a separate association with lower total AMD risk [18]. The authors could not fully explain that inconsistency.
This is dietary-intake epidemiology, not a supplement trial, so causation is not established [18]. [18]

Anthocyanins are chemically unstable — sensitive to heat, light, oxygen, pH and enzymes — which is what drove these formulations [12]. Whether they deliver better outcomes in people has not been shown.
Side Effects
- Digestive upset, tooth staining
Blueberry fruit is rated likely safe for most people in food amounts, and no serious side effects surfaced in the clinical trials reviewed [10][11].
Consumer sources report temporary tooth staining and digestive upset — diarrhoea, less often constipation — at high intake [15]. These are anecdotal reports, not controlled adverse-event data. [15]
Precautions
- Warfarin and vitamin K consistency
A cup of blueberries carries about 28 µg of vitamin K [15].
A supplement-interaction reference does not list blueberry among common warfarin-interacting supplements [16]. The American Heart Association's advice is about keeping vitamin K intake steady day to day, not avoiding it, and its examples are leafy greens rather than berries [17].
The theoretical concern is a sudden large increase blunting the drug, so consistency is the point [15][17]. [15] - Two weeks before surgery
RxList advises stopping medicinal-dose blueberry at least 2 weeks before scheduled surgery [11].

Blueberry fruit in normal food amounts is rated likely safe in pregnancy and breastfeeding by both major references [10][11].
Neither has enough data to rate the concentrated amounts in supplements [10][11]. Both advise staying at food-level intake rather than extract or high-dose powder. [10] - Berry allergy and salicylate sensitivity
A true berry allergy is uncommon but real, and can produce hives, swelling or breathing difficulty [10].
Rare salicylate-sensitivity reactions are also reported by consumer sources [15]. [10]
Interactions
- Diabetes medications
RxList rates this a minor interaction, specifically for blueberry leaf preparations, which may lower blood sugar [11].
Combined with a diabetes drug, that could push blood sugar too low, and a dose adjustment might be needed [11]. Monitoring is the practical answer. [11] - Sources disagree that any interaction exists
WebMD states there are no known interactions between blueberry and medicines, food or drink, while adding that interactions are not fully understood [10].
RxList rates the diabetes-medication interaction above as minor and advises presurgical caution [11].
Both agree the underlying evidence base is thin rather than firmly settled either way [10][11]. [10]
Food Sources
Blueberry is the food, so the question is what it contains rather than where to find it.
Per 100 g, raw blueberries give about 57 kcal, 0.7 g protein, 0.3 g fat, 14.5 g carbohydrate, 10 g sugar and 2.4 g fibre, at 84% water [20].
USDA-sourced figures for the same 100 g list 9.7 mg vitamin C, 19.3 µg vitamin K1, 0.34 mg manganese, 77 mg potassium, 6 mg calcium, 6 mg magnesium and 0.28 mg iron [21].
Blackberries are a botanically unrelated fruit, from Rubus rather than Vaccinium [24]. Per 100 g they carry more vitamin C (21 mg against blueberry's 9.7 mg), more fibre and more minerals, while blueberries hold more sugar, thiamine, riboflavin and B6.
Best Time to Take
No official guidance exists, and consistency matters more than the clock in the trials reviewed.
The one timing-specific finding is a lunchtime one. Wild blueberry extract at 111–444 mg taken around midday improved episodic memory and executive function measured 2 to 6 hours later, targeting the well-documented post-lunch dip [3].
The proposed mechanism — flavonoid-driven, nitric-oxide-mediated vasodilation — would plausibly apply at any hour, not just around lunch [3].
An 8-week powder trial split its 50 g daily dose into two 25 g packets at least 8 hours apart, mixed into water, cereal, milk, yogurt or smoothies [14]. No empty-stomach requirement appears anywhere.
No source tested morning against evening dosing.
Common Questions
Is there an official blueberry supplement dose?
No, and the sources say so directly.
RxList states there is not enough scientific information to determine an appropriate dose range for blueberry [11]. WebMD points users to the product label instead [10].
Research doses span roughly 8-fold — from 111 mg of extract to 50 g of whole-fruit powder — because the trials were testing different delivery forms, not different amounts of the same thing [3][14].
Can I just eat fresh blueberries instead?
For general intake, yes. For matching a positive trial, the arithmetic is unkind.
Anthocyanins from freeze-dried powder do reach the bloodstream intact, so fresh and dried fruit are absorbed comparably [13]. But the 50 g/day powder dose used in trials is roughly 350 g — 2⅓ cups — of fresh berries, every day [14].
And in the one head-to-head trial, the concentrated extract moved memory and blood pressure while whole-fruit powder did not [3].
How much anthocyanin is in a blueberry supplement?
There is no standard, and the range across products is enormous.
Cognitive trials have used daily anthocyanin doses from 1.35–2.7 mg in whole-fruit powder up to 7 mg in a standardized wild blueberry extract [3].
Meanwhile 100 g of freeze-dried whole blueberry powder has been measured at about 1.2 g of anthocyanins [12]. That roughly thousandfold spread is the difference between a capsule extract and a bulk-powder serving, not a labelling error.
Do freeze-dried blueberry supplements lose potency in storage?
Yes, and the finished product format matters more than time alone.
A food-science storage study reported an anthocyanin half-life of 139 days for wild blueberry powder under ambient conditions [19].
After 8 weeks in different foods, an ice pop and a refrigerated juice held over 90% of initial anthocyanin content, a refrigerated oatmeal bar 86%, and a gummy only 43–51% depending on temperature [19].
Is a blueberry actually a berry?
Yes — and it is one of the few fruits called a berry that genuinely is one.
It develops from a single ovary with a fleshy pericarp and typically multiple seeds, which is the botanical definition [22][23].
Blackberries and raspberries are aggregate fruits made of many drupelets, and strawberries are not berries either [23].
