France's ANSES reports effects observable from around 20 mg and pronounced at 50 mg, and advises against combining synephrine with caffeine at all [7].
At the permissive end, Health Canada allows 50 mg/day alone, and the Advantra Z manufacturer's own 60-day trial found no adverse cardiovascular, hematological, or blood-chemistry effects at 98 mg/day [3][4].

Bitter orange is Citrus aurantium, the Seville orange behind British marmalade and Cointreau [1][13].
The supplement is a different animal: an extract of the fruit or peel, standardized for p-synephrine, an adrenergic compound structurally close to ephedrine [1].
It went mainstream after the FDA banned ephedrine alkaloids in supplements in 2004, and now sells as the ephedra-free stimulant in weight-loss and pre-workout products [1].
It is not a nutrient and has no deficiency state. What it does have is an unusually wide regulatory disagreement about how much is too much [5].

Side Effects
- Reported cardiovascular events
Heart attacks, strokes, and abnormal heart rhythms have been reported in people taking bitter orange products [1]. One published case report describes variant (Prinzmetal) angina attributed to bitter orange in a supplement [1].

Seville orange juice carries furanocoumarins that inhibit intestinal CYP3A4, much as grapefruit juice does [15].

Food Sources
Raw, this fruit is close to inedible. Kew Gardens and NCCIH both describe the pulp as too sour and bitter to eat fresh [1][13].
It earns its keep cooked and processed: Seville orange marmalade, candied peel, and flavoring for Cointreau, Grand Marnier, Curacao, and Triple Sec, plus some beers and spirits [1][13].
Regionally it turns up in Chinotto, an Italian bitter soda, in the Maltese soft drink Kinnie, and as the TCM ingredient Zhi Shi [14].
Here is the counterintuitive part from BfR's exposure work: p-synephrine is not unique to bitter orange [5]. Sweet oranges, mandarins, clementines, and their juices carry it too, generally at lower concentrations [5].
Averaged across the population, ordinary food alone supplies about 6.7 mg/day of synephrine, rising to roughly 25.7 mg/day for heavy citrus eaters [5].
So normal citrus intake can already meet or exceed some regulators' entire supplement ceiling [5]. The extract's whole purpose is concentrating something food already provides in small amounts [5].
Inside the fruit, synephrine is highest in unripe peel (0.056% by weight), lower in the albedo (0.028%), lowest in the flesh (0.019%) [5].
Best Time to Take
No trial has tested morning versus evening, or with food versus empty stomach, for bitter orange [1].
What is documented is the pharmacology: p-synephrine peaks in blood 1-2 hours after an oral dose and has a half-life of about 2 hours, with roughly 80% eventually excreted in urine [5].
That stimulant profile is the whole basis for the usual advice to take it earlier in the day. No bitter-orange sleep study confirms or refutes it [1][6].
The better-sourced timing rule is about what you take it with. Both BfR and ANSES flag combining it with caffeine, or taking it around intense exercise, as raising cardiovascular risk beyond either factor alone [5][7].
Common Questions
Is bitter orange extract the same thing as synephrine?
Not exactly. The extract's main protoalkaloid is p-synephrine, the compound regulators write limits around [9].
Whole extract also carries octopamine, N-methyltyramine, tyramine, and flavonoids such as naringin and hesperidin [9].
Two look-alikes matter here. Methylsynephrine (oxilofrine) is a synthetic, illegal adulterant, never a natural constituent [9].
The isomer m-synephrine has distinct cardiovascular effects, and studies have sometimes wrongly attributed those to bitter orange extract [9].
Bitter orange versus grapefruit — what's the difference?
Both are citrus hybrids involving the pomelo, from different crosses [14]. Bitter orange is pomelo crossed with mandarin; grapefruit is thought to have arisen later in the Caribbean, from pomelo crossed with sweet orange [14].
In practice, grapefruit is eaten fresh or juiced, while bitter orange's raw pulp is essentially inedible and goes into peel, extract, and marmalade [1][13].
They share the part that matters clinically: the FDA warns that Seville orange carries the same furanocoumarin drug-interaction risk as grapefruit juice [15].
Does bitter orange raise heart rate and blood pressure?
Dose and duration decide it.
With prolonged use, the 2022 meta-analysis of 18 trials found significant increases in both systolic and diastolic blood pressure [6].
Single-dose studies are more mixed. One placebo-controlled trial gave 450 mg of extract, 27 mg of combined m- and p-synephrine, and saw no change in blood pressure or QT interval at 1, 3, 5, or 8 hours [6].
Another single dose, 900 mg of a 6%-synephrine extract, did produce modest but real rises in systolic and diastolic pressure and heart rate [6]. Acute effects look smaller and more variable than sustained ones [6].
How do I store fresh Seville oranges?
They are a short-season fruit, roughly late December to mid-February in the UK and Spain [18].
A commercial produce reference gives about 1 week at room temperature, 2-4 weeks refrigerated, and up to a year frozen whole [18].
